P. David Applegate, DPM
Dr. Phillip “David” Applegate is in his third year of podiatric medicine and surgery residency in Oklahoma City, OK. He completed his undergraduate studies at Texas A&M University in College Station, TX before attending podiatric medical school at the Arizona School of Podiatric Medicine at Midwestern University in Glendale, AZ. There he served as delegate to the American Podiatric Medical Students’ Association, and later went on to serve as national president of the organization during his fourth year of school. Upon graduation, he trained for one year in Cleveland at The Cleveland Clinic before moving to Oklahoma City for the remainder of his podiatric surgery and medicine residency at Surgical Hospital of Oklahoma. A proud resident of Houston, Dr. Applegate is actively working towards opening up a clinic in Houston upon residency graduation in 2016.
Dr. Peter N. Brieloff
Dr. Peter N. Brieloff is the president and founder of the Brieloff Foot Centers. Dr. Brieloff has been in private practice since 1987. He is a graduate of the Johns Hopkins University in Baltimore, MD. and the Temple University School of Podiatric Medicine. He completed a residency in Podiatric Medicine and Surgery at the FDR VA Medical Center in Montrose, NY.
Dr Brieloff is Board Certified in Foot Surgery by the American Board of Podiatric Surgery as well as Podiatric Orthopedics and Podiatric Medicine by the American Board of Podiatric Orthopedics and Primary Podiatric Medicine. When he completed the last of these board certifications in 1993, he became the youngest podiatrist to achieve all three board certifications.
Dr Brieloff is a Fellow of the American College of Foot and Ankle Surgeons; the American College of Foot and Ankle Orthopedics and Medicine; and the American Academy of Podiatric Practice Management. He is also a member of both the American Podiatric Medical Association and Maryland Podiatric Medical Association.
Dr. Brieloff is on staff at the Western Maryland Health System in Cumberland and was the Chief of Surgery of the Surgery Department of the Western Maryland Health System from 2003-2011. A native of Long Island, NY, Dr. Brieloff is married with two children, Alyssa and Corey. In addition to his family and practice, he also enjoys golf.

Ryan D'Amico, DPM
Dr. Ryan D’Amico completed his undergraduate studies at LeMoyne College in Syracuse, NY. He then attended Podiatry School at the New York College of Podiatric Medicine for 4 years, moving on to complete his 3 year residency at Western Reserve Care System from 2007 to 2010, where he specialized in Podiatric Medicine and Surgery.
Dr. D'Amico recently began his private practice and has seen tremendous growth through the aid of many of the IPED board members. He also serves the community by volunteering at the Rescue Mission as the attending physician for allopathic medical students who run a clinic named "Helping Hands for Forgotten Feet" monthly in his home town of Syracuse, NY. He looks forward to helping new practitioners start and grow by sharing his experiences and knowledge of practice management.
Dr. Craig B. Frey
Dr. Craig B. Frey completed his undergraduate studies at Washington & Jefferson College in Washington, PA. He the earned his Podiatric Medical degree from the Kent State University College of Podiatric medicine, prior to completing his 3 year surgical resident program at the Cleveland Clinic Foundation and Mercy Health in Cleveland, OH.He has since opened up his own practice, with his IPED member and speaker wife Dr. Megan L. Oltmann, in Solon, OH.
Dr. Frey serves as an adjunct faculty member at Kent State University’s Podiatric Medical School. In podiatry school, he served as President of the school’s academic fraternity and served on the board of the Ohio Podiatric Medical Student Association. He currently lectures at regional medical conferences and symposiums.
Dr. Frey is still heavily involved in his community. He serves as an ex-officio Trustee on the Board of Trustees at his Alma Mater, Gilmour Academy. He is also co-president of the Alumni Association at Gilmour Academy. He was recently elected into the Gilmour Academy Hall of Fame in 2014.
Dr. Frey has a large focus on the treatment of athletes, with an extensive athletic background. While attending Washington and Jefferson College, he played and served as captain of the collegiate hockey team. He was a member of the Team USA World Inline Hockey Team, winning both a gold and silver medal.

Stephen Hartman, B.Sc. Podiatric Medicine
Stephen Hartman is the sole proprietor of the chiropody clinic "Foot Works" that is located in Uptown Waterloo at 200 King Street South. He graduated from the Chiropody program at the Michener Institute in Toronto and has more than 20 years of experience. Stephen has received his Bachelor's degree in Podiatric Medicine from the Durham Podiatry School in Durham England. He treats patients who have over 300 different types of foot related impairments such as peripheral neuropathy, foot ulcers, calluses and corns and ingrown toe nails, which are all common foot problems in the diabetic population.
He is a board certified Pedorthist, which qualifies him to fill prescriptions for footwear, modifications, foot orthotics and related devices. Stephen Hartman is also the Chief Executive Officer for the Canadian Federation of Podiatric Medicine (CFPM). Mr. Hartman is responsible for coordinating and leading the CFPM in national and international initiatives. He has held past positions as CFPM President and Chairman of the Board. He is one of the founding board members of the CFPM.
Todd Jaramillo DPM
Dr. Jaramillo joins the Board of Trustees with a strong affinity for the podiatric profession. Todd was raised in Orem, Utah and made the choice to follow in his father’s footsteps and pursue a career in podiatry. He received his bachelor’s degree in public health from Brigham Young University where he played ice hockey for the university’s club team. He went on to Des Moines University where he became very involved in the student organizations and served as Chair of the Education Committee for the American Podiatric Medical Students’ Association (APMSA). Dr. Jaramillo is currently a second year resident at Wake Forest Baptist Health in Winston-Salem, North Carolina. In his free time, he enjoys spending time with his family and playing hockey.
David Kretch, DPM
Dr. David Kretch was born and raised in Cleveland, Ohio. He attended the Ohio State University, graduating cum laude with a BA in Psychology. After undergrad, he went to podiatry school at Kent State University College of Podiatric Medicine in Cleveland, Ohio. As a student at KSUCPM, he served as the Captain for the men’s basketball team and was an active member of several organizations while also working as an assistant for a local podiatry office. Dr. Kretch is currently a resident at Rush University Medical Center in Chicago, Illinois. He is a loyal Cleveland Cavaliers and Buckeye football fan and likes to travel in his free time.
David A. Laurino, DPM
After completing his undergraduate degree at the University of Arizona with a B.S. in Exercise and Sport Sciences, Dr. Laurino graduated from the California College of Podiatric Medicine in San Francisco. He completed two years of surgical residency at the Southern Arizona Veterans Administration Medical Center and went on to become a board certified expert in podiatric medicine and surgery.
Dr. Laurino found his home and niche with the Advanced Foot & Ankle Specialists of Arizona in 2002 and hasn't looked in the rear view mirror since. He helped his practice double in size in 4 years, and then helped it double again in size 5 years after that. Dr. Laurino has a strong interest in business development, strategy, optimization, marketing, and overall stringent practice management.
He now resides in Tempe, with his wife, Katie, and two children. When this practiced foot professional isn't caring for your foot or ankle pain, or working on the business you can find him outdoors, scuba diving, golfing, cycling, hiking, or snow-boarding.

Amy Miceli, DPM
Dr. Miceli completed her undergraduate studies at Saint Francis University and Lebanon Valley College. Prior to attending the Kent State University College of Podiatric Medicine (KSUCPM) she was a Chemistry Lecturer at Harrisburg Area Community College. At KSUCPM she served as President of the Ohio Podiatric Medical Student Association (OPMSA), Chair of the Presidents' Committee within the American Podiatric Medical Student Association (APMSA), and a term as her Class APMSA Delegate. Dr. Miceli’s residency was completed at the Crozer-Chester Medical Center in Upland, PA. Dr. Miceli is currently serving a Fellowship with the NJPPSG and assisting IPED with securing credentialing for its meetings.
Hal Ornstein, DPM, FASPS
Dr. Ornstein is Founder, Chairman and Chief Executive Officer of the New Jersey Podiatric Physicians and Surgeons Group, LLC and President and Chief Executive Officer of Podiatric Super Group Management, LLC. In addition, he is Consulting Editor for Podiatry Management Magazine and Faculty Coordinator of the four-year practice management course at the Kent State University College of Podiatric Medicine (KSUCPM), and served as Immediate Past Chairman of American Academy of Podiatric Practice Management (AAPPM). He is a Distinguished Practitioner in the National Academies, has given over 250 presentations internationally and has written, along with having been interviewed for over 300 articles on topics pertinent to practice management and serves as consultant for array of companies in the podiatry community. Dr. Ornstein was very proud to create The Ornstein Family Foundation to drive his purpose in life, to give as much as he can to as many as he can. He also serves on the Alumni Board of KSUCPM and in 2011, was inducted into the institution’s Hall of Fame. In 2009, he was honored to receive induction to the Podiatric Hall of Fame and received the Podiatry Management Magazine Lifetime Achievement Award. He is co-author of a multidisciplinary management text entitled 31 1/2 Essential Tips for Practice Management Success, a Greenbranch publication. He dedicates his life to carrying out his personal mission statement; “Every Day is an Opportunity to Create and Share Miles of Smiles”, and works to help others alter their paradigm to demonstrate that “Life Just Gets Better” by encouraging definition and achievement of success. Dr. Ornstein has been in private practice for over twenty-five years and currently serves as Medical Director of Affiliated Foot and Ankle Center, LLP with an office in Howell, New Jersey where he currently resides. He actively participates in local non-profit community run programs as well as national programs such as Yoga Gangsters, a Miami-based national non-profit organization whose mission is to empower youth by addressing the symptoms of trauma and poverty using the practice of yoga.
Peter C. Paicos Jr., DPM
Dr. Paicos is a native of Massachusetts, a second generation Podiatrist (his father retired at the age of 90). He was a graduate of the charter class of Des Moines University, College of Podiatric Medicine and Surgery in 1986. His residency training was completed at West Roxbury VAMC, followed by a clinical Fellowship at Massachusetts General Hospital. Following his postgraduate training he founded Affiliates in Foot Care, PC and was the President and CEO from 1990 until May of 2015. During his clinical practice he achieved Board certification from both the American Board of Podiatric Medicine (1992) and the American Board of Foot and Ankle Surgery (1995). His passion for limb preservation focused his career with hospital based relationship including Associate Medical Director for Winchester Hospital's Wound Healing and Hyperbaric Center, a relationship he enjoyed for close to fifteen years. He served as Service Chief of Foot Surgery at Winchester Hospital 2008-2105. Currently he is the Chief Medical Officer for the New Jersey Podiatric Physicians & Surgeons Group, LLC - a twenty group practice with thirty eight physicians.
Politically, as a medical student he served as a delegate in the American Podiatric Medical Students Association, serving as chairman his senior year. The following evolution into local state activities was a predictable course. He served on the Executive Committee of the Massachusetts Podiatric Medical Society from 1994-2007, acting as President from 2004-2006. During that time period he also represented Massachusetts at the American Podiatric Medical Association House of Delegates for many terms. Additionally, Dr. Paicos has served on numerous committees and has held various appointments in both the state society, hospitals as well as within the insurance industry.
He resides in the coastal community of Newbury, Massachusetts where he serves on the town's Planning Board and Master Plan Committee.
Daniel Phan, DPM
Dr. Daniel Phan graduated from Rutgers University (Dean’s list), and obtained his medical degree from Kent State University College of Podiatric Medicine. In addition, he graduated in the top of his class in business school (MBA, 3.88 GPA), and excelled in graduate school (MPH program). As a student, he was also a nominee for the US National Mathematics Award. After college, he interned in the U.S. House of Representative and for several Fortune 500 companies.
Prior to a career in medicine, he passed the series 7 and 63 to become an equity and derivative trader/specialist in New York City focusing on biomedical technology, high-yielding ETFs, foreign ADRs, and alternative energy. He also has experiences as a consultant for business, marketing, website development, product distribution, and product branding companies. Recently, he was the Strategic Alliance Utilization and Compliance Officer for the New Jersey Podiatric Physicians and Surgeons Group. There, he worked with the leadership to establish quality and results driven treatment protocols. On top of that, he has worked with vendors on both the local and national level. Lastly, he is a managing partner at Kratos Orthopedics Devices, LLC.
Outside of work, his community involvement includes serving as an executive board member (Community Outreach) for a non-profit political organization in Manhattan. For over 16 years, he has been a proud associate member of the Fraternal Order of Police both local and nationally. Other professional memberships include the New Jersey Podiatric Medical Society, APMA, and the Rutgers University Alumni Association.
As an educator, he has contributed to the publication of the Compendium of Podiatric Medicine & Surgery in 2012. He has written articles on practice management for Podiatry Management. In his spare time, he likes to travel and read.
Ellen Wenzel, DPM
Dr. Wenzel grew up in Arizona and attended Arizona State University where she obtained a BS in Clinical Laboratory Sciences. After working for a year in the laboratory of Scottsdale Healthcare as a generalist, she matriculated at the Scholl College of Podiatric Medicine at Rosalind Franklin University in North Chicago, IL and earned her DPM in 2007.
Following medical school, she completed a three year medical and surgical residency program at Forum Health/WRCS in Youngstown, OH. As a third year resident, Dr. Wenzel served as Chief Resident.
Dr. Wenzel is an active member of several podiatric organizations. She serves as Chair of the Insurance Committee, the CAC/PIAC Representative and the Washington L&I Representative for The Washington State Podiatric Medical Association. She is a Fellow of the American College of Foot and Ankle Pediatrics and a member of the International Association of Dance Medicine. She is also Board Certified by the American Board of Podiatric Medicine, Board Certified in Foot Surgery by the American Board of Foot and Ankle Surgery, a Fellow in the American College of Foot and Ankle Surgeons, and a Fellow in the American College of Foot and Ankle Orthopedics and Medicine.
